EXCELLENCE AWARD Nomination Ballot

Annually, the FCDA Membership is asked to showcase their programs, partnerships and activities that are running well and making a real difference in the quality of life for the persons it serves. This FCDA Award recognizes excellence in program implementation as it addresses and helps to relieve local issues and was born out of our collective networking experience that if there was a challenge to addressing local needs one of our members has already figured out a practical solution. Moreover, these solutions are often “transferrable” to other locations with some tweaking to fit the respective locale. It can provide a great starting point for local programs that avoids costly miscues and help us all be more efficient with our resources.  Additionally, if we work it right answers to inquiries and ideas about program can come back to us very quickly “in time for that important meeting or required comment.
